(“Reliant”, “we” or “us”) provides this Privacy Policy to inform you of Reliant’s policies and procedures regarding the collection, use and disclosure of Personal Information we receive from users of our websites located under http://www.reliant-tech.com (the “Services” or the “Websites”).
This Privacy Policy applies only to information about individuals, and if it is provided to us through the Websites or by other messages that relate to the ‘Reliant Technologies Online Store’, regardless of how such messages are conveyed to us (email, telephone, or by using the Fax Product Order Form) (“Personal Information”). We may include links to third parties’ websites, however, we are not responsible for the privacy practices of third parties. How we collect and use Personal Information

- Public profiles: If you are a health care provider using our products you may enter information about you and your affiliates and employees into your profile; we will make such data publicly available through a searchable directory located under(“Locate a doctor”).

This information may include, but is not limited to, your computer’s Internet Protocol address, browser type, the web page you were visiting before you came to our Websites, information you search for on our Websites and links you click while visiting our Websites. You may access and modify Personal Information about yourself
At any time, you can view, correct, update, or delete your ‘Primary Contact Profile’. Any deleted information becomes immediately unavailable for Public Profiles. However, deleted information may remain stored on our servers for some period of time after deletion, e.g. for accounting purposes or technical reasons (back-up storage). If you wish to withdraw information you entered when you signed up for newsletters, or sent in a survey form, please contact us under privacy@fraxel.com. We may choose to anonymize such information instead of deleting it, except if there is reasonable basis to believe that the information, though anonymized, can still be used to identify you as an individual.
Opt out
You may opt out from our communications to you (such as newsletters, or other promotional material) by using an unsubscribe link, as provided, or by contacting us under privacy@fraxel.com. We reserve the right to contact you with certain administrative messages regarding your account and your use of this website.
